Option 1:  Complete on the Computer

1) Fill out the YELLOW highlighted regions on the forms

    You will need Adobe Acrobat Reader

2) Print & Sign at the necessary locations

3) Submit to Athletics Office w/ transportation payment receipt from ASB Office

Option 2:  By Hand

1) Print the form using Adobe Acrobat Reader

2) Complete the necessary fields in the form

3) Sign the necessary areas in the form

4) Submit to Athletics Office w/ Transportation payment receipt from ASB Office
